Payment Options

I am currently in-network with Blue Cross Blue Shield of Tennessee, BCBS of Massachusetts, Aetna, Oscar, Osford, Cigna and United Healthcare for Tennessee residents only. For out-of-network or cash-pay services I will not bill your insurance. If requested, I will provide a superbill (an outline of the service codes, diagnosis, setting, and amount paid), which you can submit to your insurance company to request reimbursement. Reimbursement is not guaranteed. Payment is required at the time of service, and I will not accept copays. IMPORTANT: Due to state law I am unable to provide treatment to TennCare-eligible patients, including duel-eligible individuals. I may agree to treat Medicare-covered patients as an out-of-network provider on a case-by-case basis.
